This questionnaire is designed to collect information about the individuals with disabilities in the family. Interviewers ask all the questions with all the individual with disabilities one by one and fill the answers in the suitable collum. In case the individual with disabilities is not able to answer (for example, people with mental disabilities), interviewers ask the questions about that person with disabiltites with the main care giver for that person.
Individuals using these data for any purpose, whether for publication or training, are asked to please credit the funding sources.
